Job summary

Heritage Behavioral Health Center in Decatur, IL is seeking a Licensed Practical Nurse I - Integrated Care to provide nursing services within our Integrated Health Clinic, delivering primary and behavioral healthcare to patients with complex needs.

Under the supervision of the RN Program Manager, you will collaborate with LPN II, providers, pharmacists, and behavioral health staff to ensure safe, timely, and patient-centered care. Strong communication and organizational skills are essential.

Qualifications

• Active, unencumbered State of Illinois LPN license.
• Current CPR certification, or ability to obtain upon hire, with certification maintained throughout employment.
• Valid driver's license, reliable transportation, and proof of current automobile insurance.
• Preferred: experience in behavioral health, integrated care, or primary care settings.

Responsibilities

• Accurately obtain and document vital signs, including blood pressure and other key indicators.
• Administer prescribed medications (weekly or as ordered) and report any adverse reactions to medical staff.
• Monitor health status and ensure timely, accurate charting in medical records.
• Coordinate with pharmacy partners to ensure timely, accurate medication refills and adjustments.
• Facilitate referrals and link patients to appropriate healthcare services.
• Collaborate with providers and team to coordinate patient care and respond to urgent concerns.
