Heritage Behavioral Health Center- HBHC in Decatur, IL seeks a Licensed Practical Nurse I – Integrated Care to join our Integrated Health Clinic team. The role focuses on delivering primary and psychiatric care with compassion, coordinating with providers and staff to ensure safe, timely patient care.

You will work under the RN Program Manager, support care coordination, monitor vital signs, administer medications, document in records, and participate in urgent care when needed.

Qualificações

• Active IL LPN license is required.
• Current CPR certification or ability to obtain on hire.
• Valid driver’s license and proof of insurance.

Responsabilidades

• Accurately obtain and document vital signs.
• Administer prescribed medications and report adverse reactions.
• Monitor health status and chart in medical records.
• Communicate changes in patient condition to providers.
