Job description

This position provides social work services including assessment, care planning, case management, and coordination for patients and their families. The role also includes acting as a liaison between healthcare services and the community, facilitating resource referrals, advocacy, and service coordination to ensure appropriate patient placement. Qualifications & Skills:

• Master’s degree in social work with licensure as a Licensed Social Worker or Licensed Clinical Social Worker in Hawaii.


• Basic Life Support certification from the American Heart Association.


• Experience working with medical diagnoses and networking with community agencies.
