Job summary

UnitedHealthcare is seeking a Health and Social Services Coordinator to support an Integrated Care Model in Hawaii, combining field visits with remote care coordination to ensure timely, high-quality services for members.

You will assess care needs, facilitate referrals to social services, manage care plans, and communicate with clinicians, families and providers to advocate for patients’ choices and outcomes.

Qualifications

• Current, unrestricted Social Worker license or LMHC in Hawaii.
• 2+ years of social work or similar experience.
• Experience coordinating long-term care, home health, hospice, public health or assisted living.
• Proficiency with MS Word, Excel and Outlook.
• Ability to travel in the assigned region to visit Medicaid members.
• Must have internet access via landline.
• Driver's license and reliable transportation.

Responsibilities

• Assess, plan and implement care strategies individualized to each patient and directed toward the most appropriate level of care.
• Identify and initiate referrals for social services, including financial, psychosocial and community resources.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate health-related information to all stakeholders to ensure coordinated care.
• Advocate for patients and families to ensure needs and preferences are represented by the health care team.
