Job summary

CareSource is seeking a Team Lead for Integrated Care Coordination - Waiver in Ohio. The role supports day-to-day operations of Care Coordinators and manages a reduced caseload of MyCare HCBS Waiver members.

You will lead and escalate clinical and operational issues, aiming to meet regulatory, quality, and member-outcome targets. The position may require up to 25% travel to various locations, including homes and offices.

Qualifications

• Bachelor's degree in related field preferred.
• Case Management certification preferred.

Responsibilities

• Provide day-to-day oversight to assigned Care Coordinators.
• Monitor workflows to ensure timely assessments, service plans, contacts, authorizations and outreach.
• Serve as first escalation point for staff questions and operational issues.
• Coordinate care for high-risk members and those with complex needs.
• Occasional travel up to 25% to homes, offices, or other settings.
