CareSource is seeking a Team Lead for Integrated Care Coordination - Waiver to oversee daily operations of Care Coordinators and manage a reduced caseload of MyCare HCBS Waiver members. You will escalate clinical and operational issues and support achievement of program goals, quality standards, and positive member outcomes.

The role requires leading workflows, ensuring timely assessments and service plans, and coordinating care for high-risk cases with potential for travel to various locations.

Qualifications

• Directs day-to-day operations for Care Coordinators
• Manages a reduced caseload of MyCare HCBS Waiver members
• Escalates clinical and operational issues as needed
• Ensures regulatory compliance and quality outcomes

Responsabilités

• Provide day-to-day oversight and direction to assigned Care Coordinators.
• Monitor workflows and ensure timely completion of assessments, service plans, contacts, authorizations, and outreach.
• Serve as first escalation point for staff questions, barriers, and member concerns.
• Coordinate care for high-risk members and complex transition cases.
