Centerpoint Health in Franklin and Middletown, Ohio, is seeking a dedicated Integrated Behavioral Health Counselor to join our collaborative primary care team. You will provide assessment, brief intervention, counseling, and care coordination for patients with behavioral health concerns and related chronic medical conditions.

Required are a Master’s degree in Social Work, Counseling, or Psychology, and Ohio licensure (LISW or LPCC).

Qualifications

• Master's degree in Social Work, Counseling, Psychology, or related field.
• OH licensure (LISW/LPCC) required; PsyD/PhD holders must have applicable Ohio licensure.
• 2+ years of clinical experience preferred in integrated behavioral health or primary care.

Responsibilities

• Provide direct behavioral health services and screenings.
• Develop clinical summaries and individualized treatment plans.
• Offer individual, group, and family counseling and therapy.
• Coordinate care with primary care providers and community resources.
• Document in electronic health records and participate in team meetings.
