Centerpoint Health in Franklin and Middletown, Ohio, is seeking a dedicated Integrated Behavioral Health Counselor to join our collaborative primary care team. You will provide assessment, brief intervention, counseling, and care coordination for patients with behavioral health concerns and related chronic medical conditions.

Required are a Master’s degree in Social Work, Counseling, or Psychology, and Ohio licensure (LISW or LPCC).

Qualifications

• Master's degree in Social Work, Counseling, Psychology, or related field.
• OH licensure (LISW/LPCC) required; PsyD/PhD holders must have applicable Ohio licensure.
• 2+ years of clinical experience preferred in integrated behavioral health or primary care.

Centerpoint Health is an FQHC providing primary care, dental, OB-GYN, and behavioral health services. We are seeking a full-time Integrated Behavioral Health Counselor to join our team at our Franklin and Middletown, Ohio locations.The Integrated Behavioral Health Counselor provides behavioral health services as part of a collaborative, integrated health care team. The position provides assessment, brief intervention, counseling, treatment, and care coordination for patients experiencing behavioral health concerns and behavioral components of chronic medical conditions.JOB DUTIESProvide direct behavioral health services to patients.Conduct initial screenings, assessments, interviews, and clinical evaluations.Administer and score appropriate screening and assessment tools.Develop clinical summaries and individualized treatment plans.Provide brief interventions for mental health concerns and behavioral components of chronic medical conditions.Provide individual, group, and family counseling and therapy.Select and implement appropriate evidence-based treatment interventions.Monitor patient progress and adjust treatment approaches as clinically appropriate.Collaborate with primary care providers and other members of the health care team.Make appropriate referrals and assist with coordination of behavioral health and community resources.Complete timely and accurate documentation in the electronic health record in accordance with Centerpoint Health policies and procedures.Participate in case reviews, team meetings, trainings, and professional development activities.Perform administrative duties and other duties as assigned.Occasional travel throughout the state may be required for meetings, trainings, seminars, or conferences.EDUCATIONMaster's degree in Social Work, Counseling, Psychology, or a related field required.LICENSURECurrent Ohio clinical licensure appropriate to the position, such as LISW or LPCC, required.Applicants with a PsyD or PhD in Psychology must hold applicable Ohio licensure required for the position.EXPERIENCETwo years of clinical experience preferred.One year experience in an integrated behavioral health or primary care setting preferred.Experience with clinical assessment, diagnosis, treatment planning, and evidence-based behavioral health interventions preferred.SKILLS AND QUALIFICATIONSKnowledge of evidence-based behavioral health therapies and treatment approaches.Strong clinical assessment, interviewing, and treatment-planning skills.Ability to work collaboratively within an integrated health care environment.Strong oral and written communication skills.Ability to establish effective working relationships with patients, families, and health care professionals.Ability to manage multiple priorities and work effectively in a fast-paced environment.Ability to work independently and as part of a multidisciplinary team.Flexibility and adaptability to meet the needs of patients and the health center.Proficient computer skills, including Microsoft Office.Experience with electronic health records preferred.Blood-Borne Pathogens Exposure: Category 1PHYSICAL REQUIREMENTSThe physical requirements of this position may include the ability to lift up to 25 pounds and perform activities such as bending, walking, navigating steps, pushing, pulling, and squatting.
