Allegheny Health Network seeks a Behavioral Health Clinician to join the care team, focusing on improving recognition, treatment, and management of psychosocial and behavioral health needs among enrolled patients. The role involves consulting within licensed scope, collaborating with care managers, and connecting patients to higher levels of care as needed.

The BHC participates in chronic and acute disease management, utilizes behavioral principles to address health risks, and supports education

Competenze

Master's degree in Mental Health Field.
2 years experience in evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders.
Licensure: LCSW or LPC or LMFT; out-of-state transfers allowed within 60 days.

Mansioni

Member of care team; develop pathways and best practices for targeted patient groups.
Support on behavioral health and substance use topics to multidisciplinary team.
Provide short- to medium-term counseling and coordination of care within ambulatory settings.
Advocate for patients and re-engage those not following up with care.
