Job summary

Allegheny Health Network is seeking a Behavioral Health Consultant (BHC) to join its care team. The BHC provides clinical mental health consultation within licensure scope to patients referred by the care team, and supports population health by addressing psychosocial factors and chronic disease management.

As a licensed mental health professional, you will participate in daily huddles, deliver short-term counseling, triage to higher levels of care, and connect patients with community resources

Qualifications

• Master’s degree in Mental Health Field.
• 2 years experience in evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders.
• Knowledge of the relationship between medical and psychological aspects of health and disease.
• Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT); for out-of-state transfers, allow 60 days to transfer license.
• CPR – American Heart Association.
• Act 34 Criminal Background Clearance Certificate Act 33 Child Abuse Clearance Certificate Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
• Compliance with HIPAA and company privacy policies.

Responsibilities

• Member of Care Team and Population Health efforts; develop clinical pathways.
• Provides education and support to multidisciplinary team members during treatment planning.
• Provides counseling interventions in ambulatory care using brief 15–30 minute sessions.
• Triage and refer patients to specialty mental health and substance abuse services when appropriate.
• Advocacy, outreach, and community linkages to address social determinants of health.
