Job summary

FPCSN is seeking a Behavioral Health Consultant (BHC) to join the primary care treatment team in Philadelphia. The BHC provides behavioral health consultation, assessment, and brief intervention, administering screenings and connecting patients to community resources.

The role supports a trauma-informed system of care across age groups, with duties spanning collaboration with nurses and physicians and ongoing supervision. Candidates must hold an MSW, Ph.D./Psy.D.

Qualifications

• MSW, Ph.D./Psy.D. in Psychology or related field is required.
• Current Pennsylvania licensure as LCSW, LPC, or Psychologist is required.
• Strong knowledge of behavioral medicine and evidence-based treatments.

Responsibilities

• Brief Consultations with primary care providers to identify and treat mental health concerns.
• Assess clinical status of patients referred by primary care providers.
• Coordinate care with medical/social work/outpatient staff; provide referrals as needed.
• Document patient encounters accurately in the electronic medical record.
• Attend supervision and team meetings for quality improvement.
