Job summary

NOAH is seeking a Behavioral Health Consultant to join a multi-disciplinary team at Venado Valley Health Center in Phoenix, AZ. This full-time role focuses on brief, consultation-based services to primary care patients with an integrated behavioral health approach.

Ideal candidates hold a Master’s in Social Work or Counseling and are licensed (LCSW/LPC/LMFT/Psychologist). 1+ year of behavioral health consults and 3+ years healthcare experience are preferred. Bilingual English/Spanish a plus.

Qualifications

• Master’s degree in Social Work, Counseling, or related behavioral health field.
• Active license (LCSW/LPC/LMFT/Psychologist) in good standing in AZ.
• 1+ year(s) of behavioral health consultative experience.
• 3+ years in healthcare settings.

Responsibilities

• Provide brief, consultative services to patients in a multi-disciplinary team.
• Develop measurable, time-limited treatment plans.
• Serve as a resource for patients, families, and staff; coordinate with care teams.
• Facilitate communication between families and healthcare providers.
• Collaborate with community resources to implement comprehensive plans.
• Maintain timely clinical documentation and quality assurance activities.
• Participate in meetings, trainings, and outreach events.
• Perform additional related duties as assigned.
