Job summary

Neighborhood Outreach Access to Health (NOAH) is hiring a Clinical Assessor/Behavioral Health Technician in Phoenix to perform comprehensive intake assessments and develop initial treatment plans under licensed supervision. You will complete required forms, coordinate with care teams, and support patients with outreach and resource linkage.

Qualifications

• Knowledge of DSM-5 and general clinical principles and practices.
• Ability to work cohesively within the treatment team.
• Excellent verbal and written communication skills and ability to interact with diverse backgrounds.
• Ability to read, write, and speak English; Spanish preferred.

Responsibilities

• Conduct comprehensive intake assessments and complete required documentation (ROI, consents, PHQ-9, ASAM, CALOCUS).
• Collaborate with providers, case managers, and teams to support caseload management and treatment planning.
• Participate in staff meetings, treatment planning sessions, and case conferences to ensure coordinated care.
• Process Releases of Information (ROI) for prior outpatient and inpatient services.
• Provide outreach and engagement following crisis interventions.
• Deliver supportive counseling under supervision and address treatment adherence barriers.
• Connect patients with community resources and document interactions per policies.
• Contribute to quality improvement initiatives and ensure patient confidentiality.
