Kaiser Permanente Northwest is expanding its Behavioral Health Consultant team to provide integrated care for pediatric and adult patients within the Primary Care Medical Home and Specialty Care settings. The BHC will perform brief, evidence-based assessments and interventions, collaborate with PCPs, and support treatment planning across medical and behavioral health concerns in a fast-paced environment.

Qualifications

• External Candidates must have a minimum two years of post-graduate clinical experience within an integrated care delivery system.
• Internal Candidates must have a minimum two years of post-graduate clinical experience within an integrated care delivery system OR two years of post-graduate applied psychotherapy clinical experience AND completed an integrated care training/course in the past five years.
• Integrated care training/course completed within the past five years (or equivalent).
• Licensure requirements across Oregon and Washington noted; credentialing and primary work state must be met at hire with post-hire requirements for additional states.
• Ability to work in-person 100% and to collaborate effectively with PCPs and specialty teams.

Responsabilités

• Apply biopsychosocial models for patient care assessment and substance abuse screening.
• Provide time-limited interventions for a broad spectrum of health concerns in primary or specialty care.
• Educate patients and families on treatment plans and discuss psychotropic medication indications and effects with PCPs.
• Efficiently use the EHR and document during sessions while handling administrative duties for billing and compliance.
• Assess and manage suicidal and homicidal risk and coordinate with Crisis teams as needed.
• Offer consultation and training to primary care teams in mental health and behavioral medicine.
• Communicate clearly with medical providers to support integrated care plans.
