Job summary

High Plains Community Health Center seeks a Behavioral Health Provider to perform assessments, provide therapy, and deliver crisis interventions for patients referred by HPCHC providers, self or community referrals. The role supports integrated, biopsychosocial, team-based care within a 360-degree model and participates in community outreach.

BLS, two years of diverse-population experience, and licensure eligibility are required.

Qualifications

• Master's degree in social work, counseling or psychology is required.
• Colorado Behavioral Health Provider License is required.
• Eligibility to bill Medicaid, Medicare, and commercial payers.
• Current Basic Life Support Certification (BLS) is required.
• A minimum of two years of experience working with all ages, diverse and/or underserved populations is required.
• A minimum of two years of experience with brief and/or solution focused therapy as well as basic therapy techniques such as: cognitive behavioral therapy, mindfulness, or psychodynamic therapy is required.
• Understanding of psychotropic medication uses and side effects in treatment of patients is required.
• Spanish language skills are preferred.
• Experience with telehealth preferred.

Responsibilities

• Provide behavioral health assessments, therapy, and brief interventions to patients.
• Provide crisis intervention as needed.
• Provide clinical supervision of behavioral health students and/or license candidates providing care at HPCHC facilities as assigned by Director.
• Engage patients, colleagues, and community to promote whole person care.
• Collaborate with HPCHC medical staff to provide whole person care and advocate for understanding of the relationship between mental health issues and physical issues.
• Maintain proficiency in EHR and complete clinical documentation in a timely manner, defined as within 48 hrs. of clinical contact.
• Occasional travel may be required including day-to-day local travel.
