Job summary

AbsoluteCARE Medical Center & Pharmacy seeks a Behavioral Health Consultant to join its interdisciplinary care team, delivering evidence-based behavioral health interventions within an integrated care model. The role partners with PCPs, psychiatry, nursing, care management, pharmacy, CHWs, and SDOHCs to support whole-person care.

The BHC will conduct biopsychosocial assessments, develop SMART goals, provide individual and group therapy, crisis response, and referrals when clinically indicated,

Qualifications

• Master’s degree in social work or related field.
• Active, unrestricted independent clinical license in the state of practice (e.g., LCSW, LPC, LMFT, LPCC, LMHC, LISW).
• Minimum 2–3 years of clinical behavioral health experience in outpatient or integrated care settings.
• Experience with Medicaid and/or Medicare populations preferred.

Responsibilities

• Provide short-term interventions and group therapy using evidence-based modalities.
• Conduct comprehensive biopsychosocial assessments and diagnostic impressions per licensure.
• Develop person-centered treatment plans with SMART goals and update plans based on progress and clinical need.
• Deliver crisis intervention, risk assessment, and de-escalation as needed.
• Collaborate with the interdisciplinary care team to align behavioral health with whole-person care plans.
• Maintain timely documentation and meet regulatory, payer, and organizational requirements.
