Allegheny Health Network is seeking a Behavioral Health Consultant (BHC) to join the care team and support overall population health. The BHC will assist care managers, provide mental health consultation within licensure, and help manage psychosocial and medical conditions.

Qualifications

• Master’s degree in mental health field and licensure as a mental health professional is required.
• Experience with counseling techniques and behavioral health interventions is preferred.

Responsabilités

• Member of Care Team & Population Health Efforts.
• Develop clinical pathways or best practice programs for targeted patient groups.
• Participate in staff meetings and trainings.
• Provide education and support on behavioral health and substance abuse to multidisciplinary teams during treatment planning.
• Engage patients via telephone and in person.
• Provide brief therapeutic counseling sessions (15–30 minutes).
• Coordinate with families and support systems with patient permission.
• Document in the medical record alongside other team members.
