Beth Israel Lahey Health is seeking a Behavioral Health Care Manager to serve as the primary contact for patients within the Primary Care Collaborative Care Model. The role involves engagement, symptom monitoring, care coordination, and delivery of low-intensity, evidence-based interventions under clinical supervision.

Qualificações

• Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, Human Services, Public Health, or related field.
• Ability to exercise good judgment clinically, legally and ethically and to consult with supervisor as needed.
• Computer literate in standard desktop applications such as Windows, Word, and Excel.

Responsabilidades

• Serve as the primary point of contact for enrolled patients, providing outreach, engagement, education, follow-up, and treatment support.
• Monitor symptoms using validated behavioral health measures (PHQ-9, GAD-7, EPDS).
• Maintain the Collaborative Care registry and support population-based care management.
• Deliver structured, low-intensity behavioral interventions under clinical supervision (Behavioral Activation, Psychoeducation, Motivational Interviewing, Relapse Prevention, Self-Management Support, Wellness coaching).
• Coordinate care among patients, providers, and community partners to ensure continuity of care and treatment engagement.
• Complete registry activities, documentation, quality reporting, and billing-related activities within established timeframes.
