Beth Israel Lahey Health is hiring a part-time Behavioral Health Care Manager for our Primary Care/OB-GYN Collaborative Care Model in Massachusetts. You will oversee patient engagement, symptom monitoring, care coordination, and low-intensity interventions under supervision.

You will maintain registries, support population-based care, and help patients access social resources while coordinating with clinicians and staff.

Competenze

• Bachelor's or Master's degree in Social Work, Psychology, Counseling, Human Services, Public Health, or related field.
• Ability to exercise good judgment clinically, legally and ethically and to consult with supervisor as needed.
• Computer literate in Windows, Word and Excel.

Mansioni

• Serve as the primary point of contact for enrolled patients, providing outreach, engagement, education, follow-up, and treatment support.
• Monitor symptoms using PHQ-9, GAD-7, EPDS and other approved screening/outcome tools.
• Maintain the Collaborative Care registry and track symptom improvement, treatment adherence and outcomes.
• Deliver structured, low-intensity behavioral interventions under clinical supervision.
• Coordinate care and communication among patients, providers, and community partners.

Formazione

Bachelor's or Master's degree in Social Work, Psychology, Counseling, Human Services, Public Health, or related field

Strong interpersonal and engagement skills

Experience in integrated care / Collaborative Care Model

Experience working with pregnant and postpartum individuals
