Behavioral Health Group - BHG in Washington, DC is seeking a Clinical Care Coordinator to support a holistic approach to client needs across physical, behavioral health, and social determinants. The role coordinates care between the behavioral health clinician and external providers, including planning and linking to higher levels of care as needed.

The position requires licensure in one of the listed clinical disciplines and strong communication, documentation, and collaboration skills.
