INTAKE SPECIALIST - Lorain County

CareStar, Inc. is currently seeking an Intake Specialist in the Clinical Department to conduct program assessments using program and CareStar identified tools and processes. The Intake Specialist will provide recommendations for eligibility per program and CareStar guidelines.

THE POSITION

CareStar is looking for motivated individuals who:

• Complete visits to assess eligibility for specified programs and conduct safety assessments, in compliance with CareStar and department guidelines.
• Document all applicant and case-related contacts and interventions in applicants' records per agency and CareStar policy, including standards of content and timeliness.
• Process specific Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and CareStar policy.
• Provide assistance with community resource referrals as appropriate.
• Identify and report high-risk situations as required per agency and CareStar policy.
• Hold an active License as RN.
• Have 12 months of experience in home and community-based services within the past 5 years.
• Possess knowledge of Medicare, Medicaid, Home Health Care, and Ohio Home Care waiver programs (desirable).
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality related to patient information; any unlawful disclosures of confidential information outside the proper course of duty will be treated as a serious disciplinary offense.
• Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Lorain County and is responsible for covering the surrounding counties.
