Job summary

Integrated Home Care Services (IHCS) is seeking an Intake Specialist to efficiently process referrals from hospitals, SNFs, PCPs, and specialists, while verifying insurance coverage for in-home nursing, medications, and equipment. The role supports a patient-centered home healthcare model and ensures timely intake, scheduling, and documentation.

Ideal candidates will have at least 1 year of customer service experience, strong communication skills, and proficiency with EMR/EHR systems; bilingual

Qualifications

• 1 year of customer service experience.
• Ability to use various computer programs and applications, including EMR/EHR.
• Bilingual Spanish a plus.

Responsibilities

• Process incoming referrals from hospitals, SNFs, PCPs, and specialists.
• Verify insurance eligibility for services in the home setting.
• Coordinate with care teams to arrange nursing, medications, and equipment.
• Maintain accurate documentation and compliance in EMR/EHR systems.

What will you be doing:

The Intake Specialist is responsible for processing incoming referrals from hospitals, skilled nursing facilities, primary care providers (PCPs), and specialists. This position ensures the verification of insurance eligibility for services including nursing, medication, and equipment needed for patients in the home setting.

What will you need to succeed:

• Effective verbal and written communication skill
• 1 Year of Customer Service Experience
• Ability to use various computer programs and applications, IE EMR/EHR
• Ability to self-motivate and work independently
• Ability to multi-task, set priorities and manage time effectively
• Bilingual Spanish a plus
