Job summary

Housing Works Inc. is seeking an Intake Specialist to screen patients, verify insurance, enroll uninsured individuals, and manage sliding fee scale applications within the EMR.

The role supports the Patient Care Coordinator in delivering comprehensive primary care services. Responsibilities include issuing Good Faith Estimates, PRAPARE screening, Medicaid entitlements assistance, and outreach to ensure patients access needed care.

Qualifications

• Strong knowledge of HIV/AIDS, case management and entitlements.
• Ability to perform psychosocial assessments and reassessments.
• Ability to discern medical emergencies and alert appropriate personnel.
• Knowledge of HIPAA regulations.
• Willing to work evenings and weekends; onsite work preference.

Responsibilities

• Screen patients for insurance eligibility and verification.
• Register new patients and ensure daily sliding fee scale applications are complete in the EMR.
• Issue Good Faith Estimates for patients not using insurance or self-pay.
• Assist Patient Care Coordinator with medical case management services for Primary Care patients.
• Complete NYS Assistor training for Insurance Enrollment within 30 days.
• Enroll uninsured patients with insurance through NYS Market Place and ADAP applications.
• Coordinate intake screening and identify program eligibility for Medicaid and related services.
• Perform PRAPARE screening and generate MRN in the EMR.
• Schedule patient appointments and provide professional, efficient front desk coverage.

• Pay or shift range: $18 USD to $20 USD
  The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

Compensation Range: $18/hr - $20/hr commensurate with experience

Overview:

Intake Specialist will be responsible for the screening of patients including insurance eligibility and verification, new patient registration, ensure daily sliding fee scale applications for uninsured patients are complete and uploaded into Electronic Medical Record, (EMR), issuing Good Faith Estimates for patient not wanting to use their insurance or self-pay, and assisting the Patient Care Coordinator with medical case management services for all Primary Care patients.

Responsibilities:

• Complete New York State (NYS) Assistor Training for Insurance Enrollment within the first 30 days
• Assist with enrolling uninsured patients with insurance through the NYS Market Place, including ADAP applications for Health Services Clients.
• Issues Good Faith Estimates for clients who require them at least 3 days in advance of their visit.
• Verifies Insurance for Daily Visits and Walk-in Visits.
• Completes intake screening of new patients and identify program and insurance eligibility to ADHC, OASAS, Health Home, OMH, PrEP, RAD etc.
• Completes PRAPARE screening for new and annual patient visits
• Generate lead in eICare for new patients including MRN
• Complete sliding scale application for uninsured/underinsured patients and uploads into the EMR.
• Provides (PRAPARE), social determinates of health assessment tool, and referral services for all clients enrolled in Primary Care, which include but is not limited to, coordination of care and case management referrals
• Assesses primary care patients for entitlements and assists patients that need to enroll in Medicaid HMO. Assist clients with changing their insurance/ PCP.
• Participates in interdisciplinary meetings, health fairs for insurance enrollment
• Participate in Morning Huddle, regular medical staff meetings including Quality Improvement.
• Documents clinical interventions such as patient health questionnaire PHQ 2/9, in the EHR.
• Performs outreach activities by telephone, formalized written letters to enrolled patients who have not accessed services or who have not shown up for appointments for more than three months. Evidence of outreach efforts must be documented in the medical record.
• Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.
• Assist with patient registration process with professionalism and accuracy
• Performs professional and efficient scheduling of patient appointments
• Understands the needs of our customers, effectively provides service and or resolves problems, escalate to Clinic Manager or Health Center Director. Dedicated to meeting the expectations of internal and external customers, working to create a positive experience for all

• Understanding while working in a fast-paced environment with competing priorities may require assisting in other roles as needed.
• Front Desk Coverage during staff shortage or lunch break coverage.
• Other duties as assigned by either Health Center Director or Clinic Operations Manager

Requirements:

• The candidate must have a strong knowledge of HIV/AIDS, case management and entitlements.
• The candidate should be able to perform psychosocial assessments and reassessment and demonstrate the ability to discern medical emergencies and alert the appropriate medical and clinical personnel for assistance and support.
• Knowledge of HIPAA regulations on requests for medical documents is required.
• Strong organizational and clinical skills required.
• Proficiency in Spanish is preferred.
• The candidate must be willing to work on evenings and weekends.
