Ultimate Staffing in Woodbury, MN is seeking a Healthcare Intake & Referral Coordinator to serve as a central point of contact for referrals, admissions, and client onboarding. The role requires coordinating referrals, verifying insurance, EMR data entry, and communications with providers, clients, and families.

Standard business hours, onsite, full-time with temp-to-hire. Ideal candidates have healthcare intake experience, knowledge of insurance verification, and familiarity with EMR systems;

Qualifications

• Associate degree required; Bachelor's preferred.
• Experience with healthcare intake, admissions, or referrals.
• Medicare intake experience is a plus.
• Home health or hospice experience preferred.

Responsabilités

• Coordinate incoming referrals and admissions.
• Verify insurance eligibility and obtain authorizations.
• Manage client demographics, medical information, and documents in the EMR.
• Communicate with referral partners, providers, clients, and families.
• Process orders, referrals, and related documentation.
• Create and maintain client care plans and records.
• Track referral activity and client data using internal systems.
• Assist with scheduling and on-call coverage.
