You will receive inquiries and referrals, collect intake information securely, verify eligibility, collaborate with intake, credentialing, billing and quality teams, and help ensure smooth service delivery while maintaining confidentiality.

Qualifications

• Healthcare intake or case-management support experience.
• Clear communication and accurate data entry.
• Ability to handle confidential information securely and respectfully.

Responsibilities

• Receive inquiries and referrals, explain available programs, collect intake information securely, verify eligibility and authorizations with the appropriate team, and track referral outcomes.
• Work collaboratively with department leadership, intake, credentialing, billing and quality teams; maintain confidentiality and communicate service gaps promptly.
