Job summary

Enhabit Home Health & Hospice is seeking an Intake/Coordination professional to support referral processing and service transitions. You will work with branch and sales teams to ensure timely coordination and adherence to regulations. Strong customer service and mobility are required.

The ideal candidate has a health care background with knowledge of Medicare guidelines and comfort with mobile tech. A valid driver license is required for travel across locations.

Qualifications

• Must have a high school diploma or equivalent.
• Must have at least two years of experience in health care and one year in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must understand Medicare guidelines governing home health or hospice.
• Must have basic technology skills, including mobile devices.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
