Job summary

Enhabit Home Health & Hospice is seeking a referrals coordinator to manage patient referrals and support seamless transitions to services. You will collaborate with branch and sales teams to ensure coordination and align with regulatory guidelines.

The ideal candidate has a high school diploma, healthcare experience, strong customer service, and familiarity with Medicare rules. A valid driver’s license is required to fulfill field responsibilities within a nationwide home-based care network.

Qualifications

• High school diploma or equivalent.
• At least two years of experience in the health care industry.
• One year experience in home health or pediatrics.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
