Job summary
Horizon is seeking a Manager of Intake and Provider Relations to provide executive leadership for referral development, intake operations, and business development. You will drive growth in referral volume, census, and revenue while ensuring exceptional patient-centered care and market presence across southeastern Wisconsin.
The role requires RN credentials, BSN, CPR/BLS, and a proven ability to lead cross-functional teams in a hospice environment.
Qualifications
- Active RN license in Wisconsin in good standing.
- BSN from an accredited nursing program.
- CPR/BLS certification (American Heart Association).
- Valid driver's license with reliable transportation and auto insurance.
- Strong documentation skills and ability to learn electronic medical records.
Responsibilities
- Provide executive leadership for referral development, intake operations, and business development.
- Drive growth in referral volume, census, and revenue while improving customer service and market presence.
- Lead intake and referral management with defined processing times and conversion targets.
- Oversee insurance verification and authorization with focus on turnaround time and accuracy.
- Escalate and resolve provider concerns within established timeframes; monitor trends monthly.
- Develop annual growth plans aligned with strategic goals and measure against growth objectives.
