Job summary

Horizon Home Care & Hospice, Inc. in Milwaukee seeks an Intake & Provider Relations Manager to lead the Intake and Sales hub, coordinating referrals and ensuring exceptional patient and family service.

You will supervise on-site Intake RNs and staff at affiliate hospitals' intake stations, manage day-to-day sales operations, and apply knowledge of Medicare, Medicaid, Managed Care and Commercial Insurance to maximize reimbursement.

Qualifications

• Bachelor's degree required; BSN preferred.
• Five years related experience required, including at least three years home care nursing with knowledge of third‑party government and managed care.

• Effectively manage Intake process to ensure complete referral information is obtained and promptly communicated to the appropriate teams
• Supervise Intake RNs and support staff on-site as well as at affiliate hospitals' intake stations
• Manages the day to day operation of our sales team
• Demonstrate working knowledge of all aspects of home care reimbursement including Medicare, Medicaid, Managed Care and Commercial Insurances

We are in need of a charismatic and knowledgeable health care professional to direct our Intake and Sales team. Oversee day-to-day operations of the busy hub that is our Intake department.

This is a highly visible leadership role accountable for coordinating the best possible care and customer service for our patient referrals and their families.
