Horizon Home Care & Hospice is seeking a Part-time RN Intake Care Coordinator for our Intake Department at our Corporate Headquarters in Wisconsin. The role involves assessing patients, developing care plans, and coordinating admissions for home care and hospice services.

Qualifications

• Degree from an Accredited School of Nursing (BSN preferred).
• Current Wisconsin RN license.
• Minimum of 3–5 years of related experience.
• Hospice experience is strongly preferred.
