Job summary

Gesher Human Services in Detroit, MI is seeking an Intake Case Manager for the Community Inclusion program. You will recruit individuals with disabilities for Skill Building and Supported Employment services, manage referrals, tours, and intake meetings.

This role requires a Bachelor's degree in psychology, social work, or related field, at least two years of relevant experience, strong communication and time-management skills, and the ability to meet growth targets while maintaining compliance.

Qualifications

  • Bachelor's degree in psychology, social work, special education or related field required.
  • Two years related work experience serving individuals with disabilities required.
  • Criminal background must meet funding source requirements.
  • Fluency in American Sign Language skills is a plus.

Responsibilities

  • Cultivate referral relationships with schools, funders, and community partners to meet enrollment goals.
  • Respond to referral inquiries promptly and oversee tours and intake meetings.
  • Complete intake packets, develop plans, and provide interim case management support.
  • Train staff on each participant’s IPOS and document training accordingly.
  • Collaborate with leadership to ensure appropriate service placement and benefits planning.
  • Prepare and submit necessary documentation in computerized databases for funding sources.
