Job summary

CLASS Community in Pittsburgh is seeking an Intake Coordinator to welcome, register and admit participants into its services. The role involves gathering records, assessing needs, coordinating with participants, families, funders and staff about admission status, and ensuring a smooth intake process.

You will work under the Executive Director of Community Programs, coordinate with program directors on referrals, and help link candidates to appropriate services while maintaining accurate

Qualifications

• Bachelor’s degree or equivalent experience in human services.
• Admissions and case management experience preferred.
• Experience with CLASS/HHAX/EHR systems preferred.
• Proficient in Outlook, Word, Excel.
• Valid PA driver’s license and reliable transportation.

Responsibilities

• Identify viable referrals and link to appropriate program.
• Coordinate intake visits and complete intake processes.
• Document in EHR/Therap for service delivery.
• Communicate with program staff and participants about status.
• Assist with data collection for quality management.
• Coordinate with Community Engagement Specialists.
• Support program staff training.
