Qualifications

• Compassionate, detail-oriented intake professional required.
• RN or LPN license in Virginia is required.
• Experience with home health or intake coordination preferred.

Responsibilities

• Manage intake and recertification of home health referrals.
• Coordinate physician orders for home care and recertification; relay to care team.
• Collaborate with physicians and clinicians to implement and update patient care plans.
• Maintain thorough, timely patient records; communicate changes to the care team.
• Educate patients and families about care plans and agency services.
• Ensure intake processes meet agency, state, and federal standards.
• Promote safety and infection control in care delivery.
• Mentor new staff in intake techniques and compliance initiatives.
