Job summary

Enhabit Home Health & Hospice is seeking a referrals coordinator to support patient referrals and act as a resource for referral sources. Collaborate with the branch and sales teams to ensure smooth transition to services and adherence to regulatory guidelines.

The role requires a high school diploma, healthcare experience, customer service skills, knowledge of Medicare guidelines for home health, and basic technology skills. A valid driver's license and reliable transportation are required.

Qualifications

• High school diploma or equivalent is required.
• Two years of healthcare experience and one year in home health or pediatrics is preferred.
• Experience with Medicare guidelines for home health or hospice is required or strongly preferred.
• Must have demonstrated customer service experience and reliable technology skills.
• Valid driver's license and dependable transportation required.
