Job summary

CenterWell Senior Primary Care is seeking an Intake Coordinator for Home Health to support referrals, screen patients, and coordinate care transitions. The role reports to the Intake Supervisor and involves quick, accurate information exchange with referral sources and clinical teams.

Responsibilities include documenting call outcomes, identifying payor sources, obtaining authorizations, and ensuring smooth handoffs to home care services.

Qualifications

• High school diploma or equivalent.
• Typing speed 40+ wpm.
• Proficiency with Microsoft Word, Excel, Outlook, and Teams.
• 3+ years in health care delivery or related business experience.
• Knowledge of medical terminology.
• Knowledge of insurance reimbursement processes.
• Data entry experience.

Responsibilities

• Receive and respond to calls from referral sources or potential patients.
• Record outcomes and perform follow-up calls.
• Conduct initial telephone screening for home care eligibility.
• Advise branch staff and ensure smooth transition to home care.
• Identify payor sources, verify benefits, negotiate rates, and obtain authorizations.
• Access account information and communicate terms to staff.
• Collaborate with intake team to resolve payor issues.
• Monitor referral sources' satisfaction and report issues.
