Job summary

FFF Enterprises in Temecula, CA is seeking an Intake Coordinator to manage referrals for patient care services, determine eligibility, obtain authorizations, and coordinate with pharmacy, nursing and client service teams.

You will guide start-of-care paperwork, review financial obligations with patients, enter data into NuFACTOR system, and ensure compliance with company policies and regulatory requirements.

Qualifications

• High school diploma or equivalent required.
• One year health care related experience in a pharmacy, home healthcare, infusion related setting or hospital is preferred.

Responsibilities

• Accepts and processes referrals for patient care services.
• Determines patient eligibility and benefits, obtains authorizations and secures financial obligations.
• Coordinates with pharmacy, nursing and client service specialists to initiate patient care services.
• Notifies departments of new referrals and coordinates information amongst referral sources, clinicians and sales.
• Completes start of care paperwork and tracks signed copies within NuFACTOR system.
