Job summary

Elliot Health System is seeking an Intake Representative II for the Holt Ave Outpatient Rehab in Manchester, NH. This full-time, 32-hour-per-week role involves managing new patient referrals, coordinating with providers and referring offices, and ensuring accurate data entry in the Epic EMR system.

Responsibilities include coordinating the intake process, verifying demographics and insurance, obtaining records, and scheduling patients with the appropriate rehabilitation provider.

Qualifications

• High school diploma or equivalent required.
• Minimum of three years in a medical office setting.
• Experience with Epic EMR and basic Microsoft Office.

Responsibilities

• Processes all incoming referrals for new and existing patients and contacts patients to schedule appointments.
• Assists potential new patients and referring offices with the new patient process and handles high volumes of phone contact and electronic/faxed information.
• Verifies patient demographics, insurance, and history; documents information in Epic.
• Maintains logs and records to track referrals and follow up on pending issues.
• Completes MINI registrations in EPIC and schedules patients with appropriate providers.
