Qualifications

• Associate degree required; bachelor’s degree preferred.
• Experience in healthcare intake, admissions, referral coordination.
• Medicare intake experience preferred.
• Home care experience preferred.
• Familiarity with medical terminology.
• Good computer skills and EMR experience.
• Certified Medical Assistant (CMA) preferred.
• Background check required.

Responsibilities

• Coordinates referrals, admissions, medical orders, insurance verifications, and other client activities.
• Reviews, distributes, and follows up on inbound/outbound faxes.
• Inputs client information into EMR and tracking systems daily.
• Communicates with referrals, clients, families, and staff about status and care.
• Creates client care plans and maintains patient files and documentation.
