Job summary

Enhabit Home Health & Hospice in Grapevine, TX is seeking a referrals coordinator to support the transition of patients into home health services. You will coordinate patient referrals, serve as a resource for referral sources, and work with branch and sales teams to ensure smooth transitions.

The role requires strong customer service, knowledge of Medicare guidelines for home health, and basic technology skills including mobile devices.

Qualifications

• Must have high school diploma or equivalent.
• At least two years in the health care industry and one year in home health or pediatrics.
• Experience in customer service.
• Familiarity with Medicare guidelines that govern home health or hospice.
• Basic tech skills including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
