Qualifications

Job Summary

As an Intake Coordinator, you will be responsible for completing intake referral forms, answering phones, and entering referrals into our system. You will also be responsible for scheduling, authorizations, and other miscellaneous tasks to support the smooth operation of our office.

Responsibilities/essential Functions

• Answer phones and receive intake information, entering it into the appropriate form
• Take telephone referrals if no nursing staff is available and pass them on to the nurse as soon as possible
• Participate in the evaluation of field staff in regard to punctuality, reliability, and assignments
• Document and report changes in patient status, service requirements, and employee assignments
• Participate in the Quality Assurance program and attend in-service programs as needed
• Perform other duties as assigned

Requirements

• High School graduate
• A minimum of one (1) year of working experience, preferably in a healthcare setting
• Excellent communication skills
• Must have a criminal background check
• Ability to work independently and effectively in a fast-paced environment
• Proficiency in Microsoft Office and ability to learn new software

Special Equipment to be Operated

• Office equipment, including phone, computer, printer, and copy machine

Work Environment

• Indoors, performing computer/office duties and phone work
• Occasional travel to referrals and meetings, requiring transportation to various sites

Salary:

$35,000 - $45,000 ANNUALLY
