Emblem Home Health is seeking a dedicated RN to manage patient intake, referrals, and admissions for home health. You will verify insurance, coordinate services, and ensure a smooth transition of care while building strong relationships with physicians and hospitals.

The role requires an AZ RN license, patient intake or care coordination experience, and CPR certification. This full-time position offers a Friday–Monday four 10-hour shift schedule with remote options on weekends.

Qualifications

• Currently licensed as an RN in Arizona.
• Home Health experience is a plus.
• Experience in patient intake, admissions, or care coordination preferred.
• Excellent communication, customer service, and relationship-building skills.
• Knowledge of healthcare regulations.
• Current CPR certification.

Responsabilités

• Oversee all daily patient intake and referral operations.
• Manage the full intake process, including insurance verification and authorization.
• Coordinate timely patient admissions in accordance with company policies.

Full-time position offering a Friday - Monday schedule, working four 10 hour shifts. Saturdays and Sundays can be worked remotely.

Your Impact

As the Home Health RN Clinical Intake Coordinator you will perform patient intake, referral coordination, and admissions processes. You will serve as the first point of contact for patients and referral partners, ensuring timely admissions, verifying insurance, coordinating care services, and delivering a seamless transition into home health. You will:

• Oversee all daily patient intake and referral operations
• Manage the full intake process, including insurance verification and authorization
• Ensure compliance with all state, federal, and payer requirements
• Coordinate timely patient admissions in accordance with company policies
• Build and maintain strong relationships with physicians, hospitals, and referral sources
• Ensure smooth patient transitions to home care, including care coordination and education
• Track and report referral data, trends, and performance metrics
• Promote and maintain a positive reputation in the community as a high-quality provider
• Identify opportunities to improve services and patient access to care
• Maintain knowledge of contracts, payer sources, and community resources

Our Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona
• Home Health experience a plus
• Experience in patient intake, admissions, or care coordination preferred
• Excellent communication, customer service, and relationship-building skills
• Knowledge of healthcare regulations and business operations
• Current CPR certification
