Job summary

KeystoneCare Home Health and Hospice seeks a dedicated Intake Coordinator to manage referrals and educate potential patients and referral sources about services. You will entry patient data into EMR systems, assign cases to clinical teams, and ensure timely initiation of care while maintaining confidentiality.

You will collaborate with patients, families, and healthcare professionals, coordinating schedules and addressing insurance and location constraints to support seamless care transitions.

Qualifications

• 3+ years’ intake experience with a healthcare organization, preferably home health or hospice.
• Proven ability to multitask and communicate effectively.
• Ability to use electronic documentation systems and scheduling tools.
• Proficiency with Word, Excel, and Outlook.

Responsibilities

• Receive and manage referrals via phone, email, and networks.
• Educate patients and referral sources about services and capabilities.
• Assess ability to accept referrals based on location, clinical needs, and insurance.
• Enter patient information into the EMR system accurately.
• Distribute assignments to clinical teams based on needs and staff availability.
• Ensure timely initiation of patient care per regulatory timelines.
• Maintain communication with patients, caregivers, and referral sources about care initiation and changes.
• Adjust staff schedules for planned/unplanned absences.
• Support nurses to meet daily/weekly productivity goals.
• Deliver exceptional service to internal and external stakeholders.
• Maintain confidentiality of patient information.
• Perform additional duties as assigned by management.

Primary Job Responsibilities

• Receive and manage referrals via phone calls, emails, and referral networks
• Educate patients and referral sources about the agency’s services and capabilities
• Assess the agency’s ability to accept referrals based on location, clinical requirements, and patient insurance
• Accurately enter patient information into the Electronic Medical Record system
• Distribute patient assignments to clinical teams based on patient needs and staff availability
• Ensure timely initiation of patient care in accordance with regulatory and agency timelines
• Maintain effective communication with patients, caregivers, and referral sources regarding care initiation and any changes
• Adjust clinical staff schedules in the event of planned and unplanned absences
• Provide support and direction to ensure nurses meet daily and weekly productivity goals
• Deliver exceptional customer service to all internal and external stakeholders
• Maintain strict confidentiality regarding patient information
• Perform additional duties as assigned by the management team

Requirements

• 3+ years’ intake experience with a healthcare organization, preferably in home health or hospice environments
• Ability to multitask in a fast-paced environment and communicate effectively
• LPN preferred
• Strong customer service skills and a proven ability to collaborate with healthcare professionals, patients, and families
• Excellent data entry and scheduling skills
• Self-starter capable of working both independently and as part of a team
• Familiarity with electronic documentation systems
• Proficient in Microsoft Office products, including Word, Excel, and Outlook
• Proof of COVID-19 Vaccination or an approved Medical or Religious exemption
• Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
