Job summary

Addus HomeCare in Clarksville, TN is hiring a Full-Time Home Health Intake Coordinator. You will receive referrals by phone and in person, explain services, and help coordinate initial nursing assessments to support patients in staying safely at home.

Qualifications

• Must be at least 18 years old
• One (1) year of experience in home health, healthcare, or a related field preferred
• Strong organizational, communication, and customer service skills
• Ability to manage multiple priorities in a fast-paced environment
• Ability to complete documentation accurately and on time
• Valid driver’s license and proof of auto insurance required

Responsibilities

• Receive and process patient referrals by phone and in person
• Explain home health services and benefits to patients, families, and referral sources
• Assist in identifying alternative services when a patient is not eligible for care
• Coordinate and schedule initial nursing assessments and admissions
• Prepare and maintain admission packets and required documentation
• Support community outreach efforts and maintain strong referral relationships
• Provide administrative and clerical support to the clinical team
• Maintain accurate patient records and documentation in a timely manner
