Addus Home Health in Clarksville, TN is seeking a Full-Time Home Health Intake Coordinator. The role involves receiving referrals, explaining services to patients and families, and coordinating admissions to support patients staying safely at home.

You will schedule initial nursing assessments, prepare documentation, and assist the clinical team with everyday administrative tasks. A valid driver’s license is required for field-related duties and travel may be involved.

Qualifications

• 1 year of experience in home health, healthcare, or related field preferred.
• Must be at least 18 years old.
• Strong organizational, communication, and customer service skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Valid driver’s license and proof of auto insurance required.
