Addus Home Health is hiring a Full-Time Home Health Intake Coordinator in Silver City, NM. You will support patients and families by coordinating services that help individuals recover and remain safely at home.

Responsibilities include receiving referrals, explaining services, coordinating nursing assessments, and maintaining documentation. Local candidates preferred; Monday–Friday schedule 8:00 AM–5:00 PM MST. Join a compassionate team and contribute to high-quality care.

Qualifications

• Minimum age of 18 years.
• One year of experience in home health or related field preferred.
• Strong organizational and communication skills.
• Ability to manage multiple priorities in a fast-paced environment.

Key Responsibilities

• Receive and process patient referrals by phone and in person


• Explain home health services and benefits to patients, families, and referral sources


• Assist in identifying alternative services when a patient is not eligible for care


• Coordinate and schedule initial nursing assessments and admissions


• Prepare and maintain admission packets and required documentation


• Support community outreach efforts and maintain strong referral relationships


• Provide administrative and clerical support to the clinical team


• Maintain accurate patient records and documentation in a timely manner

Qualifications

• Must be at least 18 years of age


• One (1) year of experience in home health, healthcare, or a related field preferred


• Strong organizational, communication, and customer service skills


• Ability to manage multiple priorities in a fast-paced environment


• Ability to complete documentation accurately and on time


• Valid driver’s license and proof of auto insurance required
