Job summary

Heart to Heart Hospice is seeking an Intake Coordinator to manage daily patient referrals and intake operations. The role involves initiating EMR charts, maintaining workflow, and ensuring compliance with CHAP standards and payer rules.

You will collaborate with referral sources to improve service perceptions and oversee data reporting for referral trends. The position requires healthcare experience, strong customer service, and a valid driver's license.

Qualifications

• Familiarity with healthcare regulations and contract provisions.

Responsibilities

• Direct daily patient referral and intake operations, establishing intake policies.
• Initiate chart in EMR and maintain workflow.
• Maintain census, admissions, and care plan meetings on stand up board.
• Ensure compliance with state, federal, and CHAP regulations for referrals.
• Build relationships with referral sources to enhance service perception.
• Gather and report referral statistics and trends.
• Participate in strategic planning to improve services for unmet needs.
• Timely submission of all documentation.

Intake Coordinator

What You Must Have:

• Degree in health care or related field preferred
• At least three (3) to five (5) years of experience in health care, preferably in hospice operations
• Admissions/Intake and customer service experience
• Valid Driver’s License and Automobile Insurance
• Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices
• Experience with EMR software

What You Will Do:

• Direct all daily patient referral and intake operations including participating in the establishment and implementation of intake policies
• Initiate chart into EMR and maintain workflow
• Maintain census, pending admissions, red flag, journey’s end and care plan meetings on stand up board
• Ensure compliance with all state, federal and CHAP as applicable referral/intake regulatory requirements
• Establish and maintain positive working relationships with current and potential referral sources, build and monitor community and customer to enhance perceptions of Heart to Heart Hospice as a high-quality provider of services
• Gather, collate, and report referral statistics including key customer referral trends
• Participate in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs
• Maintain comprehensive working knowledge of company contractual relationships and ensures that patients are admitted according to contract provisions
• Participate in quality assessment performance improvement teams and activities
• Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources should services not be provided by Heart to Heart Hospice
• Timely submission of all documentation
• Performance of other duties as required
