Carolina Outreach in Charlotte, NC is seeking an Intake Clinician to perform comprehensive clinical assessments and connect clients with appropriate services. The role emphasizes collaboration with families and community supports to plan and implement treatment needs.

This position requires a Master’s degree in Counseling, Social Work, or a related human services field and an active North Carolina mental health license or eligible associate license.

Qualifications

• Education: Master’s degree in Counseling, Social Work, or other human services field.
• Licensure: Must hold a current mental health license in North Carolina or associate license (e.g., LCSW/A, LCMHC/A).

Responsibilities

• Conduct initial intake assessments with families and/or community agencies requesting services.
• Recommend the types of treatment modalities and refer to the appropriate programs.
• Develop/evaluate the service plan to ensure that services are being delivered and are appropriate to the needs of the child and family.
• Assist in client enrollment and authorization for services.
• Engages and maintains community partnerships with stakeholders to ensure satisfaction and update status of referrals.
• Conduct client intake assessments.
• Connect clients to appropriate services.
• Develop and monitor service plans.
• Work with families and community partners.
