Job summary

The Guidance Center is seeking an Intake Clinician to streamline the intake process for new clients, conduct comprehensive assessments, and craft individualized treatment plans. You will play a pivotal role in connecting clients to appropriate care from their first contact with our services.

Key duties include crisis and updated assessments, interdisciplinary collaboration, timely documentation, and facilitating treatment team meetings.

Qualifications

• Master's Degree in a behavioral health or social services related field.
• Arizona Level One Fingerprint Clearance Card required.
• Current Driver License with no restrictions.
• Willing to provide member transport to and/or from TGC or community appointments when needed.
• Must be at least 21 years of age.

Responsibilities

• Facilitate intake assessments and develop treatment plans.
• Provide intake, crisis, or updated assessments as needed.
• Collaborate with internal and external providers for care coordination.
• Attend and contribute to interdisciplinary case consultations.
• Document progress and clinical notes same day or concurrently.
• Work 100% onsite on the TGC campus in Arizona and adhere to essential staff status.
