Job summary

Sinnissippi Centers seeks to hire a SUPR Intake Coordinator at Dixon/Route 2 to ensure access to coordinated behavioral health care. The role provides intake and care coordination for youth, families, and adults navigating substance use and mental health services within our multidisciplinary team.

The ideal candidate has a Bachelor's degree or nursing license and experience with screening, assessment, and treatment planning, plus knowledge of confidentiality and risk management.

Qualifications

• Bachelor's degree and/or nursing license required.
• Experience with screening, assessing, and treatment planning for mental health disorders.
• Knowledge of differential diagnosis and psychopharmacology basics.
• Familiarity with confidentiality and risk management in healthcare settings.
• Ability to engage patients and build therapeutic relationships.

Responsibilities

• Provide Psychosocial assessments to youth and adults.
• Collaborate with other staff, clinicians and medical professionals in a fast-paced environment.
• Provide brief evidence-based interventions.
• Provide short, but intense follow-up.
• Connect clients to resources.
• Use outcome-based models to track client improvement.
