Springwell, Inc. is seeking an Intake Assessor to help people have their voice heard about their care needs.

This is a hybrid role with no weekends, evenings, nights, or holidays, offering flexible schedules and a compensation of $28.21/hr for a 37.5 hour work-week. Responsibilities include conducting in-person assessments for Home Care eligibility, creating care plans, coordinating services with vendor agencies, reviewing resources, and communicating with families and professionals.

Qualifications

• A Bachelor's Degree or higher is required.
• Applicants without a Bachelor's may qualify with an Associate's Degree plus 3 years of eldercare/case management experience, or by completing at least 50% of Bachelor's coursework with a plan to finish within 3 years.
• Experience using computers, data entry, and generating reports.
• Must have reliable car, valid driver's license, and clean driving record.

The compensation for this role is $28.21/hr for a 37.5 hour work-week.

• Conduct comprehensive, in-person assessments with older adults to determine their needs and eligibility for in-home services through the state-subsidized Home Care program.
• Create care plans and coordinate services with a network of vendor agencies as part of an interdisciplinary team.

QUALIFICATIONS:

• Bachelor's Degree or higher.
• Applicants without a Bachelor's Degree may be considered if:
  • The applicant has an Associate's Degree and at least 3 years' experience working with elders or providing case management services; OR
  • The applicant has completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3 year period.
• Experience working with computers, entering data, and running reports.
• Applicant must have a reliable car, current driver's license, and safe driving record.
